Are you a qualified Adults Social Worker seeking a role that makes a real difference? Pertemps are working with a supportive and forward-thinking Discharge to Assessment Team in Merseyside to find an experienced Adults Social Worker who can work collaboratively with healthcare professionals to ensure a smooth transition for patients from hospital to home or other appropriate settings.
Role Overview
As part of this innovative service, you will play a crucial role in assessing the needs of adults, facilitating discharge planning, and providing ongoing support to individuals and their families.

Key Responsibilities

• Conducting comprehensive assessments of patients’ needs and circumstances.
• Developing and implementing person-centered discharge plans in collaboration with multidisciplinary teams.
• Providing emotional support and guidance to patients and their families during the discharge process.
• Ensuring compliance with relevant legislation and best practice guidelines.
• Maintaining accurate case records and documentation in accordance with organisational policies.

Candidate Requirements

• A recognised social work qualification (e.g., Degree in Social Work, DipSW, or equivalent).
• At least 3 years’ post-qualifying experience
• Current registration with Social Work England
• Experience in adult social care, hospital discharge or D2A services
• Strong communication and interpersonal skills, with the ability to work effectively in a team
• Knowledge of relevant legislation, policies, and procedures related to adult social care
